The Phoenixville Police Department will be investigating as thoroughly as possible the burglary that occurred at your property. It is important that we have a list of all property that was stolen from you.

- Permanent Signage Request Application (PDF)
Provide the Police Department with the location, type of sign and reason for the sign.
- Person With Disability Parking Placard Application (PDF)
Once the form is completed by you and your physician, you may bring the form back to be notarized free of charge if you are a resident of the borough. Then, mail the notarized form to the Pennsylvania Department of Transportation (PennDot). Please be aware that PennDot processes this form and we have no way to track the progress of your application.

Provide the Police Department with a statement pertaining to an incident that has occurred.
- Temporary "No Parking" Sign Request Application (DOC)
Are you moving or having work done to your property that may require reserved parking? If so, we are here to try to help you with these events. Please give us at least 48 (business) hours notice and we will make up and post the signs for you. Please know the signs must be posted at least 24 hours in advance so your neighbors will know about the parking restriction. Also there is a $25 dollar fee for this service that will need to be paid at the time of your request.
